Upon inspection of his back he was noted to have a large gluteal hematoma measuring 4 cm x 4 cm on his right gluteal region. The area was severely tender to palpation. His neurologic exam showed decreased muscle strength of 2/5 at his right thigh area and 0/5 on dorsiflexion and plantar flexion of his right foot. He had no sensation to light touch on his right lower extremity below his right knee. Lab studies showed WBC: 45.5 g/dL, Hgb 15.1 g/dL, Hct 42.9g/dL, and platelet count of 456. His basic metabolic profile showed (mEq/L): sodium of 134, potassium of 6.4, chloride of 99, bicarbonate of 16, BUN of 22 (mg/dL), Creatinine of 2.87 (mg/dL) with an anion gap of 26.4. His lactate was 7.0. His liver function test showed a total bilirubin of 0.9μmol/L, AST of 159, ALT of 365. His CK was 41,154 U/L. His toxicology screening was negative for alcohol, positive for opiates and cannabinoids. The Brazilian guidelines for physical education propose that children should engage in dance, sports, gymnastics, games, and martial arts and experiment with a diversity of movements in different environments in PE classes.

Other countries have different school schedules where children may spend most of their waking hours in schools. Even though PE classes are the most active period in school, research has shown that children accumulate less than the recommended time spent doing MVPA in PE classes and in school-time as a whole. Although many reports have highlighted small proportions of MVPA in PE classes, it would also be informative to detail the specific context of the PE classes that might affect this lack of intense PA. Analyzing the contexts of PE classes might give an insight into the PA levels observed, as studies report that a great proportion of PE classes are spent in sedentary or light intensity activities, such as class management or knowledge. Despite the importance of these contexts, they should not be the main focus of PE classes. The present study addresses two important issues: first, the description of the contexts that prevail in the PE classes; second, the intensity pattern of the classes in the presence of these contexts. Information regarding these topics might be important to develop novel intervention strategies to increase PE classes’ intensity and benefit children’s motor development. Thus, the objective of this study was to describe the PE classes’ context and intensity in 7 to 11-year-old schoolchildren in a city in southern Brazil.

We used data from the CAAFE project (www.caafe.ufsc.br) entitled “Development and evaluation of a system for monitoring food intake and physical activity of schoolchildren aged 7 to 10 years - CAAFE”. The study was conducted in the city of Florianopolis, from March to May 2013. Florianopolisis the capital of the state of Santa Catarina in southern Brazil. Participants gave oral consent and their parents signed a written consent form. No financial reward was offered. The project was approved by the Ethics Committee on Human Research at the Federal University of Santa Catarina (UFSC) under protocol 2250/11. Six classes from 2nd to 5th grades were selected by the principal of each school, making a total of 30 classes. Children in Brazil who attend 2nd to 5th grade are expected to be 7-8 years old in the 2nd grade, 8-9 years old in the 3rd grade, 9-10 years old in the 4th grade and, 10-11 years old in the 5th grade. For the present study, data of twelve PE classes was retrieved, including time (in minutes) children spent on MVPA in these classes and the context they were inserted when being physically active. To measure MVPA, children wore accelerometers (Actigraph Gt3x+) for two days, but only data from the second day was used to prevent reactivity. The accelerometers measure acceleration during sampling periods named epochs which vary in length (usually between 1 and 60s). The intensity of the acceleration on each epoch is then classified by validated cut-off-points in activity intensity categories (e.g. 15s in moderate intensity, and 15s in light intensity). Children wore the accelerometers on the right hip, secured by an elastic band during school-time. Data was analyzed in 15 seconds epochs using cut-off points (2296 counts per minute for MVPA) validated for children and recommended to classify the intensity of activities equivalent to four Metabolic Equivalents (MET) or above in children. Women have accumulated more SrH than men (median men 0.867 µg·g-1 vs. median women 3.120 µg·g-1). The normal physiological response range covered by the linear part of the sigmoid curve was 0.418 - 2.950 µg·g-1 for men and 0.846 - 7.260 µg·g-1 for women. Values below that linear range were considered to denote low Sr nutritional status, whereas values above that range would indicate excessive dietary strontium exposure. Mean values ± standard error of mean (M ± SΕΜ) for trace element mass fraction ratios in normal tissue were: Zn/Ag – 32270 ± 5360, Zn/Co – 27010 ± 3720, Zn/Cr – 2650 ± 360, Zn/Fe – 10.9 ± 1.4, Zn/Hg – 27010 ± 3720, Zn/Rb – 73.6 ± 6.6, Zn/Sb – 34330 ± 6160, Zn/Sc – 46790 ± 7870, and Zn/Se – 1550 ± 170, respectively. It was observed that in benign hypertrophic tissues the Zn/Cr, Zn/ Hg, Zn/Sb, and Zn/Se mass fraction ratios were lower than in normal tissues. In cancerous tissue all investigated mass fraction ratios were significantly (p < 0.001) lower than in normal and benign hypertrophic tissues of the prostate. Finally, we propose to use the estimation of Zn/Ag, Zn/Cr, Zn/Fe, Zn/Hg, and Zn/Sb mass fraction ratios in a needle-biopsy core as an accurate tool to diagnose prostate cancer. Sensitivity, specificity, and accuracy of these tests were in ranges 96-100%, 89-100%, and 94-100%, respectively. Application of the Zn/trace element mass fraction ratios instead the Zn mass fraction is more suitable for PCa diagnosis, because accuracy of Zn/trace element mass fraction ratios does not depend on sample mass and changes in moisture content.

Among these methods the instrumental neutron activation analysis with high resolution spectrometry of longlived radionuclides (INAA-LLR) is a non-destructive and one of the most sensitive techniques. It allows measure the trace element contents in a few milligrams tissue without any treatment of sample. Analytical studies of the Ag, Co, Cr, Fe, Hg, Rb, Sb, Sc, Se, and Zn contents in normal, BPH and PCa tissue were done by us using INAA-LLR. For the first time it was proposed to use INAA-LLR to determine zinc and other trace element contents in needle-biopsy cores.
