In recent years, researchers have focused attention on high school youth who are exposed to differents types of violence with serious lifelong consequences. The studies conducted in Albania show that many children suffer needlessly from abuse and neglect. The data related to this phenomenon which have such a high impact on the health and well being, made it necessary for me to evaluate the prevalence and health consequences of the violence on teenagers in the city of Vlora too, in order to develop programs and methods for the prevention and treatment of violence. This descriptive transversal study included 350 teens from 3 high schools in the city of Vlora, one public school and two non public ones randomly selected. A structured and self-administered questionnaire was used for data collection. A p values ≤ 0,05 were accepted as statistically significant. The results showed that the prevalence of the violence on teenagers of Vlora district was 85.7%, with higher result in public schools than non public ones (p=0.0152<0.05). Males result to be more violated than female (p=0.0168). The highest percentage of the violated age was 15-16 years old with 63.3%. The physical violence was found to be present in 46.6% of the cases, psychological violence 20 %, sexual violence 16.7 %, verbale violence in 10%, economical violence 6.7 %. Psychological consequences dominated as health damage from the abuse. 65% of participants were not treated and only 5% had psychological support. The schools and government professionals should work on the awareness of the teens in order to acknowledge the violence, its forms and the wrong concepts of beating or hitting as educative. Previous research has shown a high prevalence of violent behavior through physical struggle. Pickett et al. reported that adolescent involvement in fighting in North America and Europe varied among countries, ranging from 37% to 69% for boys and 13% to 32% for girls. 

Youth violence is product of social, cultural, economic and biologic factors. This phenomenon happens in all the European countries. They are one of the causes of health imparity and social inequality, where the groups in disadvantage are in higher risks. Firstly, magnetic graphene oxide (GO-Fe3 O4 ) was prepared by chemical co-precipitation of Fe3 O4 magnetic nanoparticles on GO platelets and it was used as carrier for loading of doxorubicin (DOX), an anticancer drug. After loading DOX, in order to prevent the premature degradation of drug from GO-Fe3 O4 /DOX composite, it was coated with TiO2 , an inorganic porous coating, by using titanium (IV) butoxide, as precursor. In this way the GO-Fe3 O4 /DOX/TiO2 hybrid composite was synthesized. Then, the drug release of composites were monitored using UV-Vis absorbance technique and the results revealed that incorporation of porous TiO2 coating, enhanced the drug entrapment and decreased the amount of drug release, in which at pH 7.4, 56% and 15% of DOX was released from uncoated and coated composites, respectively. Moreover, after studying the release rate at pH 5.4, corresponds to the endosomal pH of cancer cell, the results showed that drug release is pH-sensitive, because the release rate of DOX in the pH 5.4 noticeably enhanced, compared to pH 7.4. Doxorubicin (DOX) is one of the most commonly used and most effective therapeutic anticancer drugs that has a high potency against of various cancers. In the other hand, DOX like the other anticancer drugs has severe side effects such as gastrointestinal disturbances, nausea, vomiting, hair loss, bonemarrow aplasia and congestive heart failure. As regards to the importance of this matter, there is an urgent need to find the new strategies and materials for targeted drug delivery. In targeted drug delivery systems (TDDSs), suitable carriers can be utilized to protect anticancer drugs from premature degradation in body and also minimize their side effects on normal cells. In this regard, so far a vast range of materials have been used as carrier in TDDSs for cancer therapy, such as polymeric nanoparticlesand vesicles, inorganic materials.

In this issue, I would like to mention about the main problems, needs and realities for healthcare professionals and especially for surgeons. Surgical residency is a difficult period for physicians and there are lots of problems during this period. Education and practice are the most important parts of surgical residency. When a physician starts the residency period, he/ she wants to be the most capable and active doctor of their branches. Enthusiasm lead physicians for working hard during this period butthey alsoface different type of problems. Several studies were reported in the literature that were indicating the main difficulties of education and social problems. Also lack of standardisation for education is the main problem in most of the European countries and this problem causes lack of confidence at the end of this education period. Surgical residency needs this standardisation so that surgeons would be more confident and capable after they become specialists.

After this education period, unfortunately the problems do not end. Doctors that have become specialists face different type of problem seven they are academic or non-academic. In almost all European countries, lack of academic positions make doctor loose their enthusiasm and interest in academic activities. For non-academics, it is nearly the same. When they start to work in any place of their countries, they face different type of problems. They cannot find enough devices or instruments to show their experience they have gained during their residency period. They do not get enough salaries for satisfaction. In some countries, patients have prejudice for doctors and they do not get satisfied with the health service in hospitals. After a time period, snowflakes are growing bigger and doctors are getting unhappy and reluctant.

In most of the countries, Ministries of Health do not take enough care of doctors and healthcare professionals. Bigger and comprehensive studies must be conducted in order to reveal the problems of these healthcare professionals so they will be happier and more successful doctors even they are residents or specialists.

Fibronectin as an Immunohistochemical Marker for Postmortem Diagnosis of Myocardial Infarction




Acute myocardial infarction is sometimes a diagnostic dilemma in forensic pathology. Our aim is to evaluate the efficacy of fibronectin by immunohistochemical methods for diagnosis of early myocardial infarction in heart autopsies.Three groups of cases selected from autopsied hearts submitted to the pathology department of legal medicine organization of Iran during years 2004-2007 which included 15 cases of definite myocardial infarction (positive control group), 18 cases of noncardiac death (negative control group) and 26 cases suspicious of myocardial infarction based on clinical presentation just before death, presence of marked stenosis in at least one major coronary artery and exclusion of other causes of death (suspicious group). Fibronectin staining was performed on sections prepared from all of the cases. With our proposed cut off value for fibronectin staining the sensitivity and specificity of this marker are 93.3% and 94.4%, respectively. This marker could also support the occurrence of myocardial infarction in 42% of our suspicious cases.We conclude that in spite of usefulness of this marker for detection of myocardial infarction, the results should be interpreted with caution.

Diagnosis of acute myocardial infarction is a challenging issue in forensic pathology. In autopsied hearts macroscopic findings of myocardial infarction accompanied by microscopic confirmation are the gold standards for diagnosis. However, the development of these changes requires time and in real practice we rarely are fortunate enough to see the typical cases. In most instances nothing other than significant narrowing of coronary arteries is found in favor of myocardial infarction. Use of an ancillary method is highly advocated in these situations. Application of triphenyltetrazolium chloride to the fresh myocardium, electron microscopy and some immunohistochemical markers including myoglobin, fatty acid binding protein (FABP), troponin, desmin, fibronectin, fibrinogen, C5b-9 and vascular endothelial growth factor (VEGF) are among many different diagnostic methods which have been used to address this diagnostic dilemma. The electron microscopy is not available in every pathology laboratory and requires comprehensive expertise. The triphenyltetrazolium chloride method requires fresh tissue and a great deal of suspicion at the time of autopsy. The immunohistochemical studies are, however, promising alternatives. The aim of the current study is to evaluate the diagnostic power of one of these immunohistochemical markers, the fibronectin, in suspicious cases of myocardial infarction.

Infantile Hypophosphatasia in Saudi Child. Case Report

This inborn error of metabolism is a rare inherited form of rickets or osteomalacia due to reduced activity of tissue-nonspecific alkaline phosphatase (TNSALP) caused by loss-of-function mutation within the TNSALP gene marked by low serum ALP. HPP is classified into different clinical types with a remarkable range of severity. Perinatal HPP is always fatal from profound skeletal hypomineralization. Infantile HPP presents before six months of age with rickets, failure-to-thrive and hypercalcemia occasionally, craniosynostosis or vitamin B6-responsive seizures as well as respiratory failure associated with progressive chest deformity. Childhood HPP features rickets and loss of deciduous teeth, hypoplasia and myopathy while adult HPP causes osteomalacia.

Hypercalcaemia and low alkaline phosphatase level constitute the main diagnostic laboratory abnormalities if infantile HPP. We aim to share experience.

Peer group pressure is an important determinant of adolescent nutrition. Meal convenience is important to many adolescents and they may be eating too much of the wrong types of food, like soft drinks, fast-food, or processed foods. Also, a common concern of many adolescents is dieting. Girls may feel pressure from peers to be thin and to limit what they eat. This review is an attempt to explore more on adolescent nutrition.

The United Nations’ Sub Committee on Nutrition meeting held in Oslo in 1998 concluded that more data on health and nutrition of school age children are needed to assess their scale of problem. It also believed that the scale of nutritional problems may have been previously under estimated. Traditionally, the main health indicator used by health planners has been mortality rates. Adolescents have the lowest mortality among the different age groups and have therefore received low priority. However, recent studies have shown that the prevalence of malnutrition and anemia is high in these age groups. The recent trend of recognizing adolescent girls as a separate as well as the crucial target group is heartening. Yet, sustained efforts are needed at the national level not only to develop comprehensive and innovative programmes for this segment of our population but also ensure their effective implementation. It is expected that such a holistic approach for the development of adolescent girls will gradually improve their nutritional status as well as enhance their nutrition/health knowledge and social awareness. Equipped with skills for income generation, these girls will be empowered to make decisions regarding themselves and their family, especially the children.

Optimism/Pessimism and Its Relationship with Locus of Control Among Children and Adolescents




The aim of this study was to examine the gender and developmental period differences in optimism-pessimism and locus of control. Additionally, it aimed to assess the correlation between optimism-pessimism and subscale of locus of control (internal, external, and unknown). The sample consisted (340) participants of school students (165 boys and 125 girls) enrolled randomly, from primary and preparatory schools located in Aleppo city. The participants aged between 10 to 15 years (M= 12.4 male, 12.8 female).The participants completed two measures: 1-Optimism-Pessimism Scale (OPS) 2- Connell’s locus of Control Scale for Children (CLCS-C). Findings of the study revealed that, the external locus of control were higher among girls than among boys, while no significant differences had been found in internal and unknown locus of control. Aon the other hand, finding showed that, there was significant gender differences for optimism. Boys were found to be more optimistic than girls were. Regarding the correlation between optimism-pessimism and subscale of locus of control, there was significant and positive correlation between optimism and Internal control. Additionally between pessimism and external control on one hand, and between pessimism and unknown locus control on the other hand. Furthermore, it has been found a negative and significant correlation between pessimism and internal control and between optimism and unknown locus of control.

There were 27 patients with a mean age of 47 years (from 40 to 65); 7 were aged more than 60 years-old. The human immune deficiency virus (HIV) first reared its head in the early eighties when young homosexual men (MSM) curiously began presenting with features suggestive of Pneumocyitis carinni Pneumonia (PCP) now jirovechi (PJP) and Kaposi sarcoma. These presentations were occurring mostly in healthcare centers located in the USA especially in New York and California. It was in 1982 the first cases were scientifically described. Later the virus was isolated from one of these patients with lymphadenopathy and identified in the laboratory by Gallo and colleagues. This eventually identified the HIV as the cause of AIDs. However it might be mentioned for historical purposes that as far back as the early 1900s the Simian immunodeficiency (SIV) virus the ‘relative’ of HIV that resides harmlessly in monkeys and Apes were first transmitted to the human race where they evolved into pathological species and eventually renamed HIV. 

In 1980 a male from San Fransisco the first case of AIDS recognized at the time was reported to the center for Disease control (CDC). However over time it as became evident that the virus through various means has a predilection for the female sex. Aim: The aim of this paper is to highlight this problem, consider the various causes and offer suggestions as to how the problems may be overcome. Results and Conclusion: Various studies have repeatedly confirmed that the female sex is significantly more vulnerable without a doubt to HIV infection than the male sex. There has to be a more active and aggressive approach designed towards reducing the higher incidence of HIV among the female sex. Presently the number of those living with HIV world wide is 36.9 million, however 70% of them reside in Sub-Saharan Africa [5, 6]. Of this 70% the majority are females. HIV not only affects the health of individuals, it impacts households, communities, and the development and economic growth of nations. Many of the countries hardest hit by HIV also suffer from other infectious diseases, food insecurity, and other serious problems. In most studies carried out on the prevalence and incidence of HIV it has been found consistently that females are more frequently infected than males. This cannot be over emphasized. Studies carried out in our institution correspond with studies carried out elsewhere in Africa in this respect. For example in studies carried out in our center based in sub-saharan Africa and published recently it was noted that females were much more infected than males. One such study revealed that 68.92% of the total infected study population were females and this was found to be statistically significantly higher than males (p=0<0.005). Another study showed that 73.8% of the study population were females [7, 8]. African females could also be two to six times more infected with HIV than men. Even though the vast majority of new HIV infections in sub-Saharan Africa occur in adults over the age of 25, HIV disproportionately affects young women. More than 4 in 10 new infections among women are in young women aged 15-24. 15-19 year olds are particularly at risk equating to higher HIV prevalence rates when they are older. For example, in Mozambique, HIV prevalence is 7% among 15-19 year olds but rises to 15% for 25 years olds. Likewise, in Lesotho, HIV prevalence rises from 4% among 15-19 year olds to 24% among 20-24 year olds. A review of 45 studies across sub-Saharan Africa found that relationships between young women and older men are common and associated with unsafe sexual behaviour and low condom use, which heightens their risk of HIV infection. This has been established in similar environs to ours repeatedly as mentioned earlier, since the introduction of highly active antiretroviral drugs in 1996 there has been a dramatic increase in the quality and length of life of infected individuals. However it is unfortunate that females that contribute significantly to the development of nations through the nurturing of both men and women from birth do not appear to be receiving the optimal benefits from HAART due to their increased susceptibility to HIV infection. Several reasons have been proposed for this disparity in HIV infection among males and females. This becomes ironical when in the same breath it is said men have more ‘risky’ sexual habits than women yet women are more infected.
